Application for Approval to Rent/Lease
The Application for Approval to Rent/Lease needs to be submitted to the Resort Office prior to an owner renting/leasing a site or home within Silver Lakes. The application includes Rules and Regulations of Silver Lakes that renters need to confirm they have read and agree to abide by. Print and complete the Application and submit the signed form to the Resort Office.

Electronic Voting and Notice Authorization
On July 1, 2015, legislation took effect enacting Section 718.128. Florida Statute, permitting homeowners' associations to conduct elections and other owner votes through the use of "an Internet-based online voting system." The Board of Directors believes the use of this method will improve communication, increase participation in the voting process, lessen the risk of fraud and significantly lower the ever-increasing costs of mailing the required documents.

Funding and Reimbursement Forms
The following forms are used for funding of activities and events in the Park. Please select the correct form based on your needs.
Request for Reimbursement/Payment - This form is used to generate a payment to reimburse funds for a previous purchase.
Request for Authorization for Funding - This form is used to request approval to make a purchase. It does not generate a payment.
Landscaping Maintenance Form
Resort Policy requires that you identify who is responsible for your landscaping maintenance when you are away from Silver Lakes. See explanation below.
This information can be provided directly to the Property Manager's Office using the following online form after logging in to the website:

During the summer months all owners, including those that are away from Naples, must have their lots maintained on a regular basis, which includes weed control in the flower beds and cement pads and tree pruning, along with maintaining the exterior of your storage shed.
In accordance with Article V Section 5:03, 'In the event the Board determines that a Lot or structure is not being maintained according to the subdivision standards, the Board may perform such maintenance and assess the Lot Owner for the cost thereof, which assessment shall be secured by a lien on the Lot'.
To alleviate the need to take this step, we are requesting that all owners take this opportunity to make the arrangements to maintain these areas and let us know the contractor or individual you have chosen to perform this service. This will assist with addressing any issues directly with the contractor in your absence. In the event the information below is not returned, we will proceed with Article V as stated above.

Mail Handling Form
Use this form to notify the Silver Lakes Resort Office the address you want to be used for your mail as you travel to or from Silver Lakes. When you submit the form it goes directly to the Silver Lakes Resort Office.

Memorial Garden Brick Order Form
Order a memorial brick in memory of someone who has lived in our park and is deceased. This is limited to one brick per person - no pets, please!
Mulch Order Form
Order bags of red mulch that will be delivered directly to your lot by Silver Lakes Staff.
Print and place order form and payment in the DROP BOX OUTSIDE THE RESORT OFFICE or mail form with check to the office.
Name Tag Order
Personalized Silver Lakes name tags and license plates are now ordered using the Silver Lakes Online Logo Shop. The previous Name Tag Order Form is no longer in use.

New Property Owner Form
You should submit the New Property Owner Form if you already have a Silver Lakes Community Website account as either ...
- an existing Owner who is buying an additional property
- a previous Renter who is buying a property
- Your existing website account will be changed to provide Owner access.
- Your newly purchased property will be transferred to your website account once confirmation of your closing is obtained from Collier County.
If you are a First-Time New Property Owner who did not previously have an account on the Silver Lakes Community Website, then you need to register for a new account by clicking:
Renter Registration
First Time Renters at Silver Lakes can register using the following online form. You will then receive Email messages covering important resort announcements, activity cancelations, and box office (ticket) sales information for events you'll not want to miss. You will automatically be unsubscribed from those Emails when your rental period ends.
Returning Renters who have not yet registered on the website should use the above link to register for the first time. In the future, returning renters should renew their rental information using the link below. Make sure your contact information is still accurate, update the SL Address if you are on a different lot, and provide new rental starting and ending dates for the current season.
